Start Date: May 1, 2004. $50 bonus per eligible application placed inforce as issued paid beginning May 1, 2004.

Eligible Products: All issued and placed Golden G Series Term/Graded Premium Life policies: G10, G15, G20, G25, G30

Payout: $50 per eligible application to writing agent after third issued/placed policy (first payment is $150). $50 for each eligible app thereafter with NO MAXIMUM!

Rules and Regulations:
In the event that an application is not taken/policy not placed, App Attack bonus will be fully charged back. In the event that a policy lapses during the first 12 months, the unearned portion of the $50 will be charged back.

Bonus will be paid to writing agent level. Payment will post direct to commission account automatically via EFT. If agent is not on EFT, payment will be given during monthly commission cycle.

Split Applications counted as split credit. Each agent to receive whatever the split percentage is. Example: If two agents split three applications equally, each agent will receive 1.5 credits toward App Attack promotion.
